Dentistry Referral Form

My name is Dr. Michelle Stephenson.

I have practiced in the Northwest Florida panhandle for over 20+ years. I’m a 1998 graduate of Auburn University College of Veterinary Medicine. Over the last 25+ years, I have developed a particular interest in dentistry and oral surgery and have pursued numerous hours of continuing education and advanced training nationwide. I am not a board-certified dentist. However, I know that many general practitioners have a particular lack of love for complicated dental extractions, oral tumor removals, fracture repair, etc. If this is you, then perhaps we can be of service.

Dental Training:

Dr. Stephenson has completed the rigorous 8-month Veterinary Dental Practitioners Program from the International Veterinary Dentistry Institute (IVDI), placing her among the distinguished in general practice veterinary dentistry.

Seventy hours of advanced dental techniques training in Bloomington, MN.

Advanced dental techniques CE and wet lab sessions at VMX conference, the largest veterinary conference in the world, with over 120 hours of dental training.

Responsible for creating and expanding several veterinary dental programs in Kentucky, North Carolina, and Florida from 2003 to the present.

Advanced dental techniques continuing education hours and wet lab sessions at the National Veterinary Dental Forum with over 40 hours of dental training.

Completed over 4,000 comprehensive dental cleanings and assessments with full mouth radiographs.

We operate as a dental REFERRAL-ONLY service.

I want to offer our dental referral services for canine and feline-only cases, ranging from uncomplicated minor extractions to major oral surgery.

What to tell your Clients:

Have them visit our Dental Page for basic information on what we offer and directions to our office.

Our dental consultation fee is $135.

If we plan to schedule the dental procedure immediately, we require a dental procedure deposit of $450 upon scheduling.

After receiving the referral form below, we will contact your client and schedule a convenient appointment.

We will do our best to honor your client's scheduled appointment; however, emergency cases receive priority, and sometimes, we may need to reschedule your client’s routine evaluation.

We will remind your client that their wait time may be longer than expected due to the unpredictable nature of these appointments.

If dental, oral, or skull radiographs have been taken, please e-mail a copy to dahdental@gmail.com or send your client a thumb drive/disk (if applicable).

Requirements for Non-Emergency Procedures

  • We require vaccines at least 2 weeks before the surgical procedure.
    • Dogs: Rabies and DA2PP
    • Cats: Rabies, FVRCP
  • All pets need basic bloodwork done within 6 months of the surgical procedure.

    • Advanced Patient Monitoring: EKG, SP02, Body Temperature, Blood Pressure, Heart Rate, Respiratory Rate, IV Fluid Therapy,

We do not offer exotic animal dentistry, orthodontics, or endodontic procedures. A dental referral examination will provide an estimate of costs.

Post-op Care

Each patient will receive a detailed post-op care instruction sheet. A copy of the complete medical record, dental chart, before/after pictures, copy of radiographs, evaluation, and surgical report (if applicable) will be sent to you and the referring veterinarian.

Post-op examinations will be approximately two weeks later unless otherwise specified.
